Dental Cents
Dental "Cents"
Companion Life offers its comprehensive Dental "Cents" program to groups with two to nine employees. Dental "Cents" offers employers the choice of two plans — Dental "Cents" Plan A or Plan B. All active, full-time employees working at least 30 hours per week are eligible to enroll. Employees may also include their spouses and any unmarried children who are younger than 19 years old.
As a value-added feature, Companion Life Dental plans include a Vision Discount Plan. The Vision Discount Plan is provided free of charge to all employees and dependents enrolled in a Companion Life Dental plan, including Dental "Cents," Dental by Design, and Voluntary Dental. The discount plan provider network and customer service is through EyeMed Vision Care.
Plan A includes 100% coverage of allowable charges for preventive services, 80% coverage for basic services, and 50% coverage for major services after a combined lifetime $100 deductible (all covered services) per person. Maximum contract year benefit is $1,000. Dental "Cents" Plan A also offers an orthodontics option (coverage for repositioning teeth, such as braces) for children. This option is available for groups with two or more employees who are insuring dependents.
Dental "Cents" Plan B includes 100% coverage of allowable charges for preventive services, 80% coverage for basic services, and 50% coverage for certain major services after a combined $100 lifetime deductible (all covered services) per person. Maximum contract year benefit is $1,000.
Choice is a big part of Dental "Cents." Employees may visit the dentist of their choice! Service is also an important feature of Dental "Cents." Companion Life accepts your dentist's claim forms, so the dentist office can file claims for you. Our standard turnaround time on an error-free claim is five working days. And you or your dentist can check the status of a claim seven days a week using a touch-tone phone and our Voice Response Unit.
Dentists may also use our secure Web tool, My Insurance Companion, to file claims online (ADA claims entry), verify patient eligibility and receive pre-treatment estimate status. Companion Life Dental Insureds may also access My Insurance Companion to check claim status, request a dental ID card, receive pre-treatment estimate status and more!

Class I - Preventive Services: Routine exams and cleanings (two per 12 months), X-rays of the roots of teeth, X-rays, full mouth or panorex (one per 36 months), Fluoride treatment for children under age 19 (two per 12 months), Emergency treatment for dental pain (minor procedures), Space maintainers, Sealants for children ages 6 through 15 (one per tooth per 36 months).

| Class II - Basic Services : Simple restorative services (fillings), Root canals, Teeth removal and other oral surgery, Medically appropriate anesthesia related to covered surgery, Recementation of crowns, Full and partial denture repair. |
| Class III - Major Services: Major endodontics (surgical procedures); Periodontics; dentures, bridges, inlays, onlays and all associated charges; Crowns. |
| Class IV - Orthodontia Services: No deductible, 50% coverage, $1,000 lifetime maximum, Children under 19 only, 12 month waiting period. |

| Class II - Basic Services/Six Month Waiting Period: Simple restorative services (fillings), Sealants for children ages 6 through 15 (one tooth per 36 months). |
| Class III - Major Services/Twelve Month Waiting Period: Endodontics (includes Root canals), Periodontics, Space Maintainers, Teeth removal and other oral surgery, Medically appropriate anesthesia related to covered surgery, X-rays of the roots of teeth, X-rays, full mouth or panorex (one per 36 months). |
| Major Services Which Are Not Covered: Dentures, bridges, inlays, onlays and all associated charges; Crowns, except associated with a root canal procedure performed while under this plan. |
