Our standard dental by design plans provide effective benefit solutions to fit any budget. To learn more, check out our dental by design brochure! A minimum of 25% employer contribution and 50% employee participation is required. We have included our employer application and employee enrollment form for your convenience.
You may prepare online quotes for groups of 10-50 lives. For groups over 50 lives, please e-mail proposals@companiongroup.com.

dental by design plans are developed with maximum flexibility in mind -- for you and your employee groups. Should you wish to customize one of our plans for your group, please e-mail proposals@companiongroup.com. Select the core benefit design that works for you, and then choose your option(s) on our convenient Request for Proposals (RFP) form.
A wide choice of options is available to modify:
- Deductible
- Number of Deductibles per Family
- Coinsurance
- Waiting Periods
- Contract Year Maximum
- Frequency of Cleanings/Exams/X-rays
- Orthodontia benefits (Adult Ortho available)
